Drupal 7 does not externally cache pages when we invoke hooks during bootstrap. This needs to be disabled (info).

$conf['page_cache_invoke_hooks'] = FALSE;

You rely on hook_boot for some ESI variable handling so I'm not sure what issues this will cause.

Thanks for reporting, will see how to manage to bypass this. The main problem is when we reach hook_init() a lot of globals already have been messed up with, which is annoying for what I do here.
