We have this happening when the Entity Reference View Widget is exposed on the add to cart form as fields on a drupal commerce line item entity.

Is there any reason why this module will cause problems when the widget is not on a normal entity or node form? Should we be choosing 'commerce line item' instead of 'commerce product' as the entity? Gah!

FYI there are multiple entity ref view widgets exposed, however we tried using just one and the problem persisted

Perhaps try the patch at #1820278: Notice: Undefined index: in entityreference_view_widget_validate - it deals with nested entityreference fields, and might be same problem here... not sure.

The 1.x codebase is no longer maintained because it was of low quality.
The 2.x branch is a complete rewrite, feel free to try it instead.
