I keep getting the following error on Form tabs.

Drupal\Component\Plugin\Exception\InvalidPluginDefinitionException: The "_____" entity type did not specify a "_________" form class. in Drupal\Core\Entity\EntityTypeManager->getFormObject() (line 223 of /app/web/core/lib/Drupal/Core/Entity/EntityTypeManager.php).

Does your entity type specify a form for that mode in its annotation?

no. the form mode and associated form are exported configs.
I was able to recreate this on a test content entity (node)

any thoughts?

That's a known core issue. If you define a custom form view mode, to use it on an entity you need a form class in the annotation.
