I noticed you use the preprocess op of hook_captcha()
this will soon be dropped, as discussed in #463002: Drop preprocess op of hook_captcha?

This can be easily replaced with more standard Drupal solution, see for example how I fixed the reCAPTCHA module in #469594: use #process instead of preprocess (CAPTCHA 6.x-2.x API update)

It seems that the CAPTCHA module has a bug conflicting with Egglue. The update has been postponed for now.
