Image alt and title attributes are 'undefined' [#401356]

If the alt attribute is not set for the original image, the inserted image's alt attribute is set to 'undefined'.

Here is my first attempt at a fix. alt and title attributes of the inserted image are now blank if values cannot be found. This also provides a fix for #365552: Why the js didn't use the title image content?.
