Trivial distributed authentication related fixes

While setting up a new (soon to be Drupal 5.0.0 based) site for testing, I found out that the distributed auth links have URL coding problems as well as some numeric array indexes are written as strings. These three trivial fixes are included in this patch. For immediate approval.

This might be a problem in 4.7 too, I have not checked.
