All users, including user1 has lost ability to access the modules page. I also don't see any new permissions listed.

The administer site configuration site permission is also on.

Perhaps if it's relevant, I'm using table prefixes?

Table prefixes wouldn't have anything to do with it.

Obviously something went very wrong in the install or something else is goofy, because this module doesn't change user1 settings at all.

I cannot reproduce this issue, so what I would suggest is:

Remove the module (the actual files).
If you can get to the module page, go there, otherwise you might have to rebuild your menus - run update.php if you need to.
Reinstall the module
Enable the module
You should then have new permissions which you can set.
