Simple menu_hook issue. The menu type is LOCAL_TASK which is only appropriate when there is a parent DEFAULT_LOCAL_TASK. Patch will remove this and set a default. The menu title was also updated to more accurately describe the entity (Payment Transaction) that is being structured.
