Using Admin module 6.x-2.0-alpha3 & 404 Blocks 6.x-1.x-dev modules. First i thought it could be my theme. After messing around it seems if your logged in and have access to the admin menu navigating to a 404 page will cause a WSOD. Error file says "forms/includes/common.inc on line 1582".
Shutting off 404 Blocks fixes the problem. Not sure if this an issue with admin or 404. Posted issue on both module issue pages. link to issue on the admin modue: http://drupal.org/node/669294
