Patch for configurable titles, link, crumbs, and urls

This patch adds several configuration options to make this module look and feel more like standard modules, and fixes an issue with international category names:

• Name and visibility of module link (at the top of the window), and also titles of all sections on module page and module block, are configurable.
• Breadcrumb can be set to include the module name, but exclude current category name, just like everywhere else in Drupal.
• Can choose to use more standard urls (with node numbers in them) instead of friendly ones that contain category names. This fixes an issue with international category names.

Note, that with default settings the module will still behave just like in the original distribution :-)
