Assuming you've setup everything the correct way, how exactly do you go about generating and creating a new sub-site with the multisite manager?

Do you just point the browser to the http://www.mydomain.com/site/my_new_site and *if* it doesn't exist, it creates the site?

The whole point of multisite-manager is that it allows a regular drupal user to create a new drupal instance. In the Create Content link, there will be a 'Drupal Site' content-type which the user can create.

No, it doesn't auto-create a site upon a web request. I imagine that could be very dangerous....
