I think it'd be best to account for both separately, as potentially one or the other could be a bottleneck.

This patch moves time reporting for crawling into millennium_fetch_records_via_bookcart(), and separates time measuring for node processing from crawling time.

Committing to 6.x-2.x branch.
Needs porting to 5.x-2.x branch.
