Add optionwidgets_onoff support

This is a follow up on the discussion at http://drupal.org/node/701622 to support single value on/off checkboxes as well.

Attached a patch that does just that and also:

- added Doxygen style comments for hook implementations
- cleaned up code formatting, moved hook implementations to the top
