Hi, I'm not sure that this is the right place to report this, and I'm assuming that you already know about this issue, but here goes anyways, just in case...
I've been frustrated with this issue for a few weeks now. Whenever I create a new issue on drupal.org, the file attachment fails when I submit the issue and I have to attach it in a follow up message. Here's how I usually proceed:
1. Create a new issue, fill out all appropriate categories, title, message and file attachment fields.
2. Preview the issue, everything looks good, hit "Save".
3. The file attachment gets lost and I have to attach a second time, in a new comment.
I haven't noticed if this behavior is consistent or not, I'll try attaching a file to this issue, just to test.

dboulet commentedOk, file attachment worked that time. In that case I'm not sure exactly how to reproduce this issue.

Anonymous (not verified) commentedI sometimes have the same problem; when I try to attach a file, the JavaScript code seems to freeze, and never complete the upload. I tried to add a screenshot to a issue report, bu I failed 9 times out of 10.

avpadernoIs there any news on this? Does anyone know what could be causing this problem?

dboulet commentedExperienced this bug again today, is this not happening to more people?

dddave commentedIt happens from time to time. I usually notice it when I come back to an issue and see that the attached pics I referred to are missing from the OP. On a regular basis I stumble upon other issues where the second post is from the OP "re"attaching the files.
Here http://drupal.org/node/768752#comment-2840390 I noticed that the files were missing in the second post and reattached them.

dboulet commentedHaven't noticed this issue at all since the site redesign, I'm going to assume that this was fixed.
Feel free to re-open this issue if problem reoccurs.
