jeremy commentedHave you followed the directions found in documentation/INSTALL.txt, and reviewed documentation/README.txt?
There is no concept of "categories" in the ad module. There are ad groups, and there are channels. It's not clear to me which you are using, if either. The simplest method of categorizing ads is to use ad groups, so I recommend you review that in the README.
To determine which version of the ad module you're using, go to Administer >> Site building >> Modules, then find the Ad module. It should say a version number there.

AngEngland commentedSorry about that - it's the ad channels. We used ad groups to set up banner ad blocks in the side bar. These appear in the side bar on every page of the website. When I enable ad channels and try to select ad images to appear only in certain channels, they do not appear AT ALL in any pages of the website, never mind in the pages I'm trying to target. :-P
Is there a link to the readme?
The version is 6.x-2.1

jeremy commentedI recommend first updating to the latest release, as it contains the latest bugfixes.
As for documentation, it's included with the ad module. Read the README in the root directory, which points you to the files in the documentation/ directory.
Finally, please carefully follow the directions in DEBUG.txt, posting the results here.

lrwebks commentedDrupal 6 is EOL and no longer supported. Closing this as outdated for that reason. Thanks for your contribution!
