Academia.edu has a great response block at the bottom of their pages: http://academia.edu/

It says "How happy are you with Academia.edu right now?" and has 10 buttons, 1-10 to respond. Clicking on it sends AJAX and turns the buttons into a thank-you. There's also a box to type comments.

Could this module offer something like this? Maybe it's worth a new module (http://drupal.org/node/547194)?

Yes, but as mentioned in the other issue you referenced to, this depends on #351698: Use entities for Feedback messages.
