Is is possible to add some kind of hack to realname module to make it ignore any incompatible modules? It seem to me that some modules - particularly those involved in account creation / synchronisation like gallery module - may never be compatible with realname.

You have to file issues in those modules to use theme('username') instead of raw usernames.
