When I just installed image assist module, I could not add any custom sized image using FCKeditor. Right after installation there is no such a choice - "Custom" - in selectbox in the popup window.
It was rather hard for me to find the problem. Because on some drupal installations I had this option, on some not. The problem is solved by saving image assist setting right after you install module.

I think this is a bug, because module installer should set default settings during the installation process.
