I'm aware of how the pathauto settings are to be set up. I do not have [lang] in the pattern setting.

I've upgraded a site from 5.x to 6.x. All new nodes are created with the appropriate path. But I can't get rid of the language prefix in old node paths without manually removing the prefix from the table row.

I think in D5 you had to include the language prefix in the alias. In D6 its added automatically and not part of URL aliases. That would explain why it's not happening for any of your new aliases.
