I have activated the module and assigned permissions to the respective user roles. However if I log in as a user with one of those roles I do not see the notify menu option. Any idea why?
Also can you give me the direct path to it. e.g. /user/x/notify.

It's a local task (sub-tab) on the /user/x path. If you still don't see it, check using a core theme to make sure it's not a deficiency in your theme.
