IIf you have fresh install of drupal and just enabled taxonomy_access module you will be able to add term permissions but this will not have effect unless you enable the module. but when you go under setting menu you will not find taxonomy_access menu link !!
You need to enable any module that have a link under setting like profile module. Then you will be able to access the taxonomy_access like to enable it.

Are you sure you use latest taxonomy_access.module for 4.6?

keve commentedTo refresh the menu cache, go to admin/menu and try to disable/enable a menu item.
