Update focus state on buttons

Problem/Motivation

1. Primary buttons have a focus state with a white border. When the button is on a white background focus state can't be seen.
2. Primary foreground buttons have a focus state with a blue border. When the button is on a blue background the focus state can't be seen.

Proposed resolution

Update the focus state to have an outline color that isn't any of the possible background colors.
