Problem/Motivation

As #3154310: Add local task menu item for available entities was committed, the route conditional_fields.tab does not exist any more so wu have to update it in the hook_entity_operation()

Proposed resolution

Replace "conditional_fields.tab" to "conditional_fields.tab.node".
