The DvG base theme doesn't override the default file_entity.tpl.php. When you include files through ckeditor it will output an visually-hidden H2 which is bad for semantics and accessibility.

Add a file_entity.tpl.php withouth the H2.

(if you don't use the DvG as a base theme, you will need to do this in your own theme)
