Hi, here's a patch which fix the missing template name.
There's a new template definition in admin page, and the problem is resolved if you click on save button on admin.
But if not, the info_template value is missing.

Thanks for the patch. It needs a slight improvement, because the legacy template is only supported when the consent method is "consent by default". I'll tweak your patch and have a new one ready in a few minutes.
