Resolve #3001926: Add a configuration form at Admin > Store > Configuration > Affirm first to ensure we have a settings form. Only add the JavaScript to any page if that checkbox is enabled.

Only include the storeName, currency, orderId, and total parameters on the checkout complete page for confirming the transaction.
