Go to admin >> configuration >> People >> captcha module settings >> captcha examples.

In CAPTCHA Points Tab, There is a list of Basic Forms given. You can enable captcha from there.
Or there is also an option for add Captcha Point For Particular FORM ID.

I am able to get the list of forms from #2. Hence closing this issue for now. Please feel free to open it again.
