window upload open not correct form and fckeditor not work and others

drupal 5.7 with Image 5x-1.x-dev and Image assist 5x-2x-dev;
1-
with default editor:
if I create a page I have two icon for to open popup image assist (they are identically), think this isn't correct;
2-
when is opened the popup and one click on upload, isn't activate inner popup the form for to make the upload but I see the the page (yes, inner popup is replicated all the page); so isn't possible to upload images;
3-
with fckeditor also if I insert the file img_assist_fckeditor.js (from dfckeditor module) inner image_Assist folder the popup of image assist is all blank; pheraphs this is an error fck file;

4- question, but with only image I can inserto only one image in the post?
