flag access for new nodes doesn't take into account the nid being set to 'new'

Same issue as #2579905: Flag access for new comments not working .

We check for:

    if (empty($entity_id) || !($node = $flag->fetch_entity($entity_id))) {

but earlier in the code we may have set the ID ourselves to 'new'.
