On some (not all) nodes, some users (anon & auth) get following error:

PDOException: SQLSTATE[23000]: Integrity constraint violation: 1062 Duplicate entry 'nodeaccess_nodereference_cache_needs_clear' for key 'PRIMARY': INSERT INTO {variable} (name, value) VALUES (:db_insert_placeholder_0, :db_insert_placeholder_1); Array ( [:db_insert_placeholder_0] => nodeaccess_nodereference_cache_needs_clear [:db_insert_placeholder_1] => b:1; ) in variable_set() (Zeile 991 von ***/includes/bootstrap.inc).

Where can i start the search?

nodeaccess_nodereference_cache_needs_clear is the key of a variable (using variable_get and variable_set)
I'm not aware of any reason why using variable_get and variable_set would cause this, but if it does then it's something core should protect for.
