PHP Notice when conditional targets component on page 2+

Define a conditional which acts on a component that is on any page after page 1. A PHP warning will be issued about a missing index. This is because the validation code looks within the form element's '#options' list by component id to get the component name, but the components are nested in option groups by page number in this list.
