If your using a different language the issue collector has a locale param:
https://[...].atlassian.net/...issuecollector-embededjs.js?locale=en-UK&collectorId=a123456f

When you enter a url like this no Issue Collector is displayed.

This is because the regex in jira_issue_collector.admin.inc, doesn't take account of this extra param.

As a simple work around, if your using a english dialect, you can remove the locale param like -
https://[...].atlassian.net/...issuecollector-embededjs.js?collectorId=a123456f

This should be fixed through #2416467: Unable to determine URL for JIRA Issue Collector script from the code. Please verify that you inserted the correct value..
