The title in the outgoing relationship (in the foreign key function) is:

'title' => t('[@module] @table references @ref_table ($ref_column)', array('@module' => $table['module'], '@table' => $table['name'], '@ref_table' => $definition['table'], '@ref_column' => $referenced_column)),

The ($ref_column) should probably be @ref_column, but even then it's not very useful. My custom table has 3 taxonomy term references, and with this set-up I can't see the difference.

I changed 'label', 'title' and 'help' to this:

$label = $definition['table'] . '.' . $referenced_column . ' (from ' . $column . ')';

because it's much shorter and it's all the information you need.

I love the module. This is what I use to Viewsify all my custom tables now. If only Entity and Views and DB talked to each other slightly better....
