For every file I add with the media browser widget to a file field an operations column is added for every row.

A patch to fix the duplicate headers. Test coverage for this will be added in #1026562: Write functional tests for adding Media to a node's single-valued media field and #1026566: Write functional tests for adding media to a node's multi-valued media field.

Tested #1 with an existing site and confirmed that it fixed the duplicate column being added when adding additional files. Working on writing tests for the previously linked issues.

Committed #1 to Media 7.x-2.x.
