Apologies if I've missed something basic, but where should an unregistered user enter the email address they'd like to receive notifications at upon leaving a comment? Comment_notify appears to be properly installed on my site, and works great for registered users with email addresses embedded in their profiles.

But after installing this module on my site, unregistered users are prompted at comment time only for their comment subject, body, "save my comment information for next time" (courtesy of comment_info module) and "notify me of follow-up comments posted here." Nowhere are unregistered users prompted to enter their email address.
