I think that you might have forgotten to rename the 'data' column to 'conditions' in the install file. The module tries to insert into 'conditions', but the install file creates a 'data' column which isn't used.

sutharsan commentedThe module is kind of fresh. Try todays version, this should be better.
Uninstall the module before updating (and/or delete the headerimage tables), I did not provide an upgrade from the yesterdays version. I did not expect any users yet ;)
