This module is really handy, but its usage of the theme layer seems really strange, which makes working with it quite difficult — the preprocess functions, theme hooks and theme templates are all implemented oddly. Is there any specific reason?

Yeah..this is super duper nasty, we 100% agree!

We reckon that using the theme system to render non HTML formats isn't actually that great, and are going to move away to a different rendering system that will still allow changes to be made, but not requiring weird use of preprocess functions and such.
