add project links, fix breakage on issue subscription form.

attached cleans up some small problems with the form, and also adds links to the project pages.

the %s fix to the form wasn't quite right. $nids can be empty, and we need to account for that. also the radio button should properly default to 'None' if there's no entry in the DB.
