Database columns 'content_type' and 'content_id' renamed to 'entity_type' and 'entity_id'

Description:

The following table fields have been renamed on {flag}, {flagging}, and {flag_counts} tables:

* content_type -> object_type
* content_id -> object_id

(Note that some of these tables have also been renamed: see change record http://drupal.org/node/1718490.)
