Is there a way to populate a hidden field with a value from a cookie? An affiliate ID for example.

There isn't an easy way to do this, but it can be done with the additional submit PHP code under the advanced options.

Does anyone know how to do this?

I find GET and SERVER variables to be pretty useless for inserting into hidden form widgets. COOKIE values and also the values returned from PHP functions such as mktime() would be very handy for inserting unique ID's.
