The path to the icon is:
http://domain/node/56/modules/filefield/ico/doc.png

It should be:
http://domain/modules/filefield/ico/doc.png

I'm not sure why you opened a new issue when you already knew about the existing one, but anyways it's fixed now (see issue #146784) and I'm closing this issue as duplicate.
