I have a content type configured to upload documents using FileField. The "Description" option is enabled. I set up Automatic Nodetitles on the content type to generate the node title using the token replacement value for the "Description". Works great, except it shows the html entities in the title instead of i.e. the apostrophe or ampersand. Guessing this would work if there was a -raw token available?

I updated the FileField module this morning after reading http://drupal.org/node/1018850, but it didn't resolve this issue.

Yeah, this is "by design" in D6, since a "-raw" token is what you're requesting. This probably isn't going to be added to the module at this point.
