After selecting Create Content - Shift, Fields for Date, Start, End, Sign-up Status, Capacity, Memo are not shown. Only shows Menu Settings, Authoring Information, Publishing Options, URL path setting. Possible a permissions issue? I believe I have all the permissions enabled.

This sounds like an issue with the CCK field permissions--what role are you trying to create shifts as?
