Block Showing two email fields

Mailchimp block showing two separate email fields and a "last name" field when in the configuration of the list I request only Merge Fields I only request Email and Name. Is this the way it's supposed to be?

Roles
anonymous user

Merge Fields
Email Address * Email

First Name
Name

All other fields I left as "Select"
