All the CC buttons are provided including PD, but no button for Copyright (All rights reserved). Is there a reason not to include this button? Would you accept a patch for this button?

It appears they don't have a button for it: http://creativecommons.org/about/downloads
Websites such as Flickr do have these buttons obviously.

I'm okay with including an all rights reserved graphic. The only reason there wasn't before is that Creative Commons didn't offer a button for it.

There are still some other buttons/icons missing so those will need to be created first as well before it could be committed.
