Site builders might not want to allow contacts to change.

Add settings to crm_relationship_type to allow contact_a and/or contact_b to not change after being set.

This change is limited to the form for now. There is still a need to allow merging contacts.

Data model changes

Default is read-only. Once the contact is set, it cannot be updated.
