Facebook not working - The access_token provided is invalid.

I found this because none of the other instagram feed modules seem to work. I got that working but my facebook page will not work. I have tried https://facebook.com/[page name], [page name], https://facebook.com/[page id], [page id] and any other combination I can think of but always get "The access_token provided is invalid." - credentials are correct and app is live. I am trying to get it to work for an organization page but I tried with my personal profile and that doesn't work either.

Either something is missing in the documentation (perhaps on what settings are needed in the facebook app api) or this isn't working.
