The icons displayed for the rendered files do not have any alt text attached. Alt text is a necessity for accessibility concerns, and is currently mandatory for most if not all government websites.

Hi!

Do you think the best would be to put the file extension in the alt attribute? or something else?

Probably the most easily implemented version would just have "file type icon", or some such, as the alt tag. If you have time to come up with alternative text for all possible files, that of course would be preferable, but IMO what I just gave describes what purpose the image serves, while being much quicker to implement.
