Thanks Earl for this simple and clever module. It makes great use of the CTools fields part, and looking at the code is a great for learning !

What about removing the Panels dependency and make it optional, so that other developers can use the provided forms without Panels ?

Fair enough. Commented out the Panels dependency in the .info file and added a README.txt explaining.
