Releases for Dynamic Cache

Resolves issues identified by rjacobs in #1940038: Numerous php warnings/notices appear to be linked to this module (ob_flush failed to flush buffer, headers already set, etc.), related to invalid bootstrap hijacking occurring in certain scenarios (see issue thread for details).
